Care Level 2 - $120

  • Burn (mild)  *includes burn dressing and topical cream if needed
  • Dog/Cat Bite *includes wound irrigation/bandaging (Tetanus booster additional cost)
  • Sprain or strain  *includes wrap
  • Strep Throat  *includes throat swab 
 

Care Level 3 - $150

  • Eye Foreign Object abrasion evaluation *includes numbing medication and eye stain for corneal abrasion evaluation
  •  Ear wax removal *Includes irrigation
  • Nosebleeds *Includes nasal packing, medication administration, and Rx if needed
  • Simple Foreign Object Removal (Splinter, Glass, Metal, etc.) *Ear, Eye, Nose, Skin
  • Simple Laceration Suturing (basic stitches)
 

Care Level 4 - $200 - $250

  • Abscess Incision and Drainage
  • Complex Sutures (multiple layer, or long/wide/deep)
  • Complex Wound Care
  • Fiberglass Splinting
  • Toenail Excision

Note: the above lists are not exhaustive and are meant to give clients an idea of what we offer and pricing. We do provide care for items outside of the above lists.

Additional Costs:

Medication administered on-site:

Sometimes, additional medication(s) may need to be given onsite. This cost will vary depending on the medication and the amount needed. These costs will always be discussed with the patient beforehand. 

 

For example: Johnny was bitten by a neighborhood dog.  He is treated at Care Level 2 and is prescribed an antibiotic. To complete his treatment, he requires a tetanus booster. This booster would be an additional cost. 

 

Additional Labs:

Occasionally, additional labs may need to be drawn to properly diagnose our patients.  The majority of our in-house labs that are not already included in the care level pricing are $20 per lab.  

 

Labs that must be drawn and sent to an outside laboratory for processing, will be charged at cost + an additional $15 service fee. These costs will be discussed at the time of service. 

 

Note: For labs sent out, you may choose to use your insurance, as accepted by the laboratory. 

As always, if you would prefer to use an area lab instead of our office, we are happy to send the orders to the lab of your choosing. 

  

Additional Patients:

During your office visit, if there is/are additional patient(s) with similar symptoms they can be added at a discounted base rate of $65 per additional patient. However, the provider must be notified at time of booking so that time slots and patient history review can be scheduled accordingly. 

 

Additional patients that do not have similar symptoms need to be booked individually, and will be charged at the full rate, based on care level.

 

Follow-up Visits:

These visits are directly related to the previous visit. Many follow-up visits can be done via Telehealth. These are charged at a discounted rate of $35. 

However, if your condition requires an in-office follow up visit (e.g. suture removal), the fee is $60.

      – Medicare patients may join our “opt-out” waiting list

$50/month young adults 18-24 with valid college ID

$25/month under 18 (with adult membership)

 

Enrollment Fee:

In order to provide great service to our Care Members, we will limit our patient panel to approximately one-third of that of a traditional practice. We will open a new set of slots every month moving forward so that each new member can get our best!

TeleHealth - $55

Your TeleHealth visit includes a virtual video or telephone consultation, follow-up instructions, and medication prescription (if needed) sent to the pharmacy of your choice.

 

Additional Costs:

Additional Patients

During your TeleHealth visit, if there is an additional patient with the similar symptoms in the household, they are billed at a discounted rate of $40 per additional patient.  Provider must be notified at time of booking so that time slots and patient history can be reviewed accordingly. Additional patients that do not have similar symptoms need to be booked separately/individually and are charged at the full rate. 

 

Example of similar symptoms: Mom and child are both experiencing flu-like symptoms. 

 

 

Follow-up Visits:

For virtual patients that require a follow-up visit (directly related to their previous visit/condition), their follow-up appointment fee is reduced to $35.

 

House Calls - Care Level Price + $50

Update: Now that our office is open, house-calls will be provided at the providers discretion on a case-by-case basis. 

House calls include travel within our main service area, medical history review, vital signs, follow-up instructions, and medication prescription (if needed) sent to the pharmacy of your choice.

 

Medication administered on-site:

Any labs drawn that need to be sent to an outside laboratory for testing will be charged at cost + an additional $15 service fee.

 

Note: You may choose to use your insurance for outside labs, as accepted by the laboratory. As always, if you would prefer to have your labs drawn at an area lab instead, we will send the order to the lab of your choosing. (Clients that prefer to use their insurance for lab expenses often choose this path.)

  

Additional Patients:

During your home visit, if there is/are additional patient(s) with similar symptoms they can be added at a discounted base rate of $65 per patient. However, the provider must be notified at time of booking so that time slots and patient history review can be scheduled accordingly. 

 

Additional patients that do not have similar symptoms should be booked individually, but during the same house-call visit, and will be charged at the full rate, depending on their care level, minus the travel surcharge.

 

Follow-up Visits:

These visits are directly related to the previous visit. Many follow-up visits can be done via Telehealth. These are charged at a discounted rate of $35. 

However, if your condition requires a house call follow-up visit (e.g. suture removal) within our regular service area, the fee is $100.

 

House Calls outside of our regular service area:

House Calls may be accepted outside of our regular service area. These are accepted and priced on a case-by-case basis. Please call or email us for pricing.
